New Patient Finance Options
We are pleased to inform you that our office participates and accepts numerous insurance plans. They include but are not limited to Delta Dental, Metlife, Aetna, Banner Health, BCBS, and Humana. Should you have a question on your current insurance, please give us a call at (480) 288-2003 and we'll be more than happy to verify your insurance. Additionally, as a courtesy to our patients, we will gladly bill your insurance on your behalf to maximize your dental benefits.
Patients without dental insurance:
Dr. Kim is pleased to announce an in office insurance plan for those without any dental insurance. The intention is to provide you with the same dental services afforded by insurance plans at a discounted rate to help absorb some of your out of pocket costs.
The details of the plan are:
Dr. Kim's Saving Plan
Includes all necessary x-rays, including full mouth series, panoramic and bitewing x-rays.
Comprehensive oral examination, periodontal screening (gum disease), oral cancer screening, oral hygiene instructions.
Recall exam (usually 6 months after the initial exam), any necessary x-rays.
Two professional cleaning per year (every 6 months), extensive cleaning does not apply.
Two emergency office visits per year.
30% discount on our usual and customary fees for all procedures.
Receive all the following above services for $209 per person, $379 for couples and $649 for the entire family (up to 6 family members) per year. Please contact our office if you have any questions or need more details about the program.
We accept all major credit cards; MC, VISA, AMEX, DISCOVER.
We also offer financing up to 12 months without interest through CARE CREDIT on approved credit.
